POSITION SUMMARY
We are seeking a dedicated Director of Health and Wellness to coordinate and oversee health and supportive services provided to residents at the Community. Responsible for the day-to-day clinical operations of the community to ensure residents’ healthcare needs are met. Provides training, supervision, and monitoring of the nursing team in the administration of direct care within the community. Performs ongoing assessment/observation of residents’ physical and psycho-social needs and coordinates with other department to assure quality and proactive care. Interacts directly with employees, residents, resident family members, vendors and professional contacts within the community.
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES
All responsibilities are to be performed within the scope of practice per state regulations.
- Fosters a non-clinical, resident-centered approach to the planning and delivery of services to residents, supporting residents’ rights to participate in decision-making and treating residents with dignity and respect.
- Maintain resident, employee, and community information in strict confidence (with community management or other employees as necessary).
- Ensure a stable health services staff of optimal quality and quantity by recruiting, hiring, supervising, evaluating, counseling, and, when necessary, terminating employees.
- Ensure regular competency evaluations of health services employees are completed.
- Perform all responsibilities related to delegation of nursing tasks (in states or provinces in which delegation is utilized).
- Perform staff education duties, including but not limited to:
Educating and training employees through conducting new employee general orientation and regular education and training classes, including all requirements of federal and state or provincial regulations.
Ensure health services employees are educated and trained using Resident Care Manager, Resident Assistant, Memory Care, Charge Nurse, and Medication Assistant Resource guidelines.
Develop, schedule, and conduct continuing education and training for health services employees and other employees as necessary.
- Assist Executive Director in developing and implementing Quality Assurance program for in-service training.
- Complete quality assurance audits
- Complete or assist with completing Nursing Assessments and auxiliary evaluations of residents and all other required and/or appropriate evaluations, addressing physical, emotional, social, and spiritual needs.
- Attend Service Plan Meetings with the Resident Care Managers.
- Completing monthly work schedule for all Nursing Department.
- Review, approve and submit timecard information for all of the Nursing Department
- Develop or assist with developing service plans, assuring health services issues, as well as other issues that potentially impact the health of residents, are effectively addressed.
- Schedule and ensure quarterly Pharmacy and Physician meetings.
- Delegates nursing tasks as allowed under applicable nursing laws and practice acts, communicating regularly with residents, their families, and their health care practitioners.
- Seeks out additional expertise and advice when needed to assure residents’ needs are met and proper practices followed.
- Participates in the marketing efforts of the management team by representing the residence in a positive manner to outside agencies and participating in community social functions.
- Monitor MAR:
Ensure medications and treatments are administered as ordered.
Ensure parameters for administration of PRN medications and treatments and assure they are entered on MAR.
Ensure appropriate diagnosis for medications and treatments are entered on MAR.
- Monitor alert charting, especially to assure appropriate employee response to issues.
- Monitor Shift Change Notes & Communications Log.
- Complete review of all Resident Incident Reports.
- Effectively utilize and coordinate services with home health, hospice, and other agencies.
- Ensure health services employees adhere to all medication room systems.
- Investigate controlled substance count discrepancies.
- Effectively communicate with residents, their family members, and other concerned parties.
- Ensure compliance with all federal and state or provincial regulations pertaining to health services.
OTHER JOB DUTIES AND RESPONSIBILITIES
- Adhere to all community policies and procedures.
- Adhere to dress/appearance code. (Changes in attire may be permitted for special circumstances as directed by your supervisor).
- Perform all assigned work for the agreed-upon wage, accepting no additional payment or gifts from residents, vendors, or others.
- Promote resident advocacy and demonstrate excellent customer service skills.
- Serve as a role model for other employees by displaying a responsible, cooperative, respectful, and positive attitude.
- Represent the community in the community at large in a professional, courteous, and friendly manner.
- Demonstrate ability to perform duties under time constraints and deadlines.
- Attend education and training classes as required by state or provincial regulations.
- Perform other duties as assigned.
QUALIFICATIONS
- Must be a Licensed Vocational or Registered Nurse holding a valid California nursing license in good standing
- Minimum of 2 years working in geriatric nursing, including experience with medication administration, delivery systems and pharmacies.
- Experience with applicable California state regulations and survey knowledge a plus
- One year in supervisory/management position, with experience with budgets, staff development, training and scheduling. Additional course work in management an asset.
- Demonstrates basic computer knowledge and ability with an aptitude to learn company applications.
- Ability to creatively problem-solve in both resident care and employee management.
- Pass a Criminal Background and fingerprint check.
- Has compassion and a special interest in working with the senior population.
- Ability to work a flexible schedule, including weekends, holidays and overtime, to meet the requirements of the position.
- Ability to relate positively and communicate appropriately with residents, families, community members, volunteers and other employees.
- Maintains clean, neat, comfortable, safe environment for residents, staff, and visitors.
- Ability to read, write, speak and understand English
- Meets all mandatory health requirements by California State regulations.
- Ability to work independently (with minimum supervision)
- Maintains current Food Handlers, First Aide, and CPR certification.
- Maintain current professional license (RN or LPN/LVN)
Actual pay rate will be calculated based on relevant years of experience and education/certification requirements.

Pay: $115,000.00 – $125,000.00 per year
