POSITION SUMMARY
Reporting to the Administrator for the Hospice, our hospice is looking for a Full Time Community Outreach Director for our Hospice Corporate office. The Community Outreach Director is responsible for increasing patient census, managing referrals, and overall promoting our growing hospice to new members. You will be critical to act as the liaison between families in need and the hospice to determine best next steps for them.
ESSENTIAL DUTIES AND RESPONSIBILITIES
In addition to all components of the above position summary the following “Essential Duties and Responsibilities” are essential to successful performance in this position
- Ensure patient census is exceeding company expectations.
- Assist prospective patient/family in making informed decisions regarding Hospice care via assessment and education.
- Participate in community and organizational programs, making presentations and joining as a member where appropriate, to facilitate a broader knowledge of the Hospice philosophy and to promote Comfort Life Hospice services.
- Initiates ongoing contact with a prospective patient via telephone, personal visits, and home visit activities as appropriate to facilitate the process.
- Contribute to team effort by being flexible in work assignments; by furnishing support; by taking initiative; and by understanding how this position affects and compliments all other hospice positions.
- Promote an atmosphere which allows for the privacy, dignity, and well-being of all patients.
- Promote positive public relations with patients, family members, and healthcare providers.
- Quality measure documentation, support, and follow up.
- Attends local meetings to represent our hospice in a marketing capacity.
SUPERVISORY RESPONSIBILITIES
The incumbent in this position has no supervisory responsibilities
EDUCATION/EXPERIENCE/CREDENTIALS
- Bachelor’s degree preferred, but a history of successful senior living placement or hospice marketing work experience is a perfect substitute
- History of exceeding sales quota
- Licensed Clinical Social Worker (LCSW) or Licensed Vocational Nurse (LVN) Required
- Ability to multi-task; simultaneously talk on the phone, take notes, and leverage internal support team a must.
- Strong computer and typing skills necessary, including Word and Excel.
- Demonstrated leadership skills to promote and develop quality assurance expectations throughout a large organization.
- Ability to be detail-oriented, follow-up on concerns, and understand systems which help ensure quality of service and accurate record keeping.
- Current driver's license and proof of insurance required.
- Licensure/Certification – Employees are responsible for maintaining individual certifications as required by job function or by law and provide verification and recertification when requested by management.

Pay scale : $100,000 to $110,000 DOE / year
