POSITION SUMMARY
As our Home Health Community Outreach Director, you will be responsible for the preparation and smooth transition of new patients into the organization. You will communicate with prospective patients, physicians, discharge planners, assisted living social workers and case managers.
ESSENTIAL DUTIES AND RESPONSIBILITIES
In addition to all components of the above position summary the following “Essential Duties and Responsibilities” are essential to successful performance in this position
- You will work individually supervised by highly driven outgoing team members
- You will visit 6-10 hospitals, skilled nursing facilities, nursing homes, assisted living facilities etc. a day
- You will build relationships and effective rapport with physicians, office staff, discharge planners, skilled nursing facilities and other referral sources.
- You will communicate and or meet with patients and families to discuss our services
- You will spend 90% of your time on the field marketing, coordinating admissions and building a network of relationships.
- You will build relationships with home health staff and ensure effective communication with referral sources
- You will serve as a liaison with hospital planners to ensure smooth transition from hospital to home
- You will be responsible for community home health program development. In cooperation with the Administrator and Corporate Marketing team, you will assess the needs of the Home Health program and develop a marketing plan of action with quarterly goals.
- You will participate in the planning for expansion of patient referrals.
- You will develop and deliver health educational services.
- You will collects all necessary paperwork required for closing and distributes the paperwork to the necessary stakeholders
- You will work in conjunction with the other team members to ensure accurate reporting of admissions and census growth
SUPERVISORY RESPONSIBILITIES
The incumbent in this position has no supervisory responsibilities
EDUCATION/EXPERIENCE/CREDENTIALS
- 2 years experience in healthcare sales
- Bachelor’s degree preferred
- Familiarity with health care reimbursement
- Knowledge of Medicare and Medicaid regulations
- Effective interpersonal skills in relating to physicians, other health specialist in the community, management and co-workers both over the phone and in person
- Strong organizational skills and attention to detail.
- Proficiency in CRM software and Microsoft Office Suite.
- Ability to work independently and as part of a team.
- Creative thinking and problem-solving abilities.
