Summary:
The Clinical Liaison is a professional nurse whose primary function is providing hospice education to the community and building relationships. The Clinical Liaison is deeply knowledgeable about the hospice benefit and can provide a comprehensive overview of services offered by the interdisciplinary team, levels of care, and hospice philosophy. The Clinical Liaison builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
ORGANIZATIONAL STRUCTURE

Education:
- Graduation from an accredited School of Nursing. Completion of an accredited Nurse Practitioner Program which conforms with the Board’s educational standards set forth in the California Code of Regulations Section 1484 or certification by a national organization/association whose standards are equivalent to those set forth in the California Code of Regulations Section 1484 -OR-
- a registered nurse who has not completed a nurse practitioner program of study which meets the Board of Registered Nursing’s educational standards as specified in the California Code of Regulations Section 1484. -OR-
- Bachelor’s degree in Nursing from an accredited program by the National League for Nursing. -OR-
- Graduate of an accredited practical nurse or vocational nursing program.
Credentials:
- Current nursing licensure in California
- CPR certification.
- Certification in Hospice and Palliative Nursing desired.
Experience:
One (1) year as of professional nurse experience and a minimum of one (1) year hospice or palliative care experience.
Other: Valid driver’s license with an automobile that is insured in accordance with state and/or agency requirements and is in good working order.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
1. Serves as a clinical resource for consultation and education to members of the interdisciplinary team, other healthcare practitioners in the community, and patients and families regarding palliative and hospice philosophy and care.
2. Establishes, builds, and nurtures relationships with staff, healthcare practitioners, and community partners to spread awareness of the agency’s hospice program.? Assist with patient’s transitioning of care to hospice when health practitioners request hospice services. This could include a hospice evaluation and/or admission.
3. Provides education on advance planning for community partners and patients.
4. Explains and completes election of benefits process with patient, family, and/or legal representative.
5. Maintains knowledge of and compliance with current Medicare/Medicaid, state/federal rules and regulations for hospice services
6. Ensures compliance with the Medicare conditions of participation and other state regulations govern the provision of healthcare.
7. Complies with all Health Insurance Portability and Accountability Act (HIPAA) requirements in accordance with federal, state, and organizational policies.
8. Assumes responsibility for personal growth. Develops, maintains, and upgrades professional knowledge and practice skills through attendance at seminars, and conferences and participation in continuing education and in-service classes.
9. Fulfills the obligation of requested and/or accepted assignments.
10. Demonstrate knowledge in communication in dealing with end-of-life issues.
11. Other duties as assigned from time to time.

Rate: $80-$125k
Mon-Fri 8:00am-5:00pm
