CARE GIVER – ASSISTED LIVING
JOB DESCRIPTION BASIC PURPOSE OF POSITION:
The purpose of the Care Giver position is to assist assigned residents with their activities of daily living in accordance with the resident’s needs and services plan and/or established expectations for assistance.
HOURLY, NON-EXEMPT PROFESSIONAL REQUIREMENTS
1. High school diploma or equivalent.
2. Must commit to the Mission and Core Values of the organization and promote facility in a positive manner.
3. Experience working as a Care Giver preferred.
4. Knowledge of standards of practice in long term care for assisting individuals requiring assistance with activities of daily living.
5. Must adhere to and carry out all applicable policies and procedures of the organization.
6. Must be able to read, write, speak, and understand the English language.
7. Must possess legible handwriting.
8. Must possess the ability to provide excellent customer service to residents, family members, visitors, facility employees, government agencies/personnel and the general public.
9. Must maintain confidentiality of verbal and written information pertaining to residents, personnel, and facility operations consistent with current regulations and acceptable standards of practice.
REPORTS TO: Charge Nurse, Assisted Living Director
DUTIES AND RESPONSIBILITIES
1. Greets new residents and orients them to their room, and ensures they are comfortable.
2. Transports residents within the Community as needed.
3. Creates and maintains an atmosphere of warmth, positive emphasis and calmness.
4. Participates in and receives daily change of shift and clinical reports.
5. Assists in keeping resident apartment clean and orderly.
6. Collects specimens (ie, urine, sputum, stools, etc.) as instructed.
7. Assists residents in preparing for activities and social programs, appointments and daily meals.
8. Encourages socialization among residents.
9. Assists residents with activities of daily living, including, but not limited to oral care, toileting, dressing/undressing, hair care, nail care, shaving, etc.
10. Assists residents with oxygen, ostomies and other medical treatments as necessary and appropriate.
11. Assists with lifting, turning, moving, positioning and transporting residents into and out of beds, chairs, wheelchairs, etc. as needed and using proper body mechanics.
12. Measures and records temperatures, pulse and other vital signs as required.
13. Assists with the care of the dying patient; provides post-mortem care as instructed.
14. Reports the presence of pressure areas, skin breakdowns, skin tears and all injuries of an unknown source.
15. Reports any known or suspected falls by residents to Charge Nurse.
16. Serves food trays and snacks in resident rooms, if necessary and appropriate.
17. Provide on-on-one support/visitation to assigned residents on a weekly basis or as requested by supervisor or as specified in the assistance plan.
18. Monitor health, safety, and well-being of residents.
19. Respond to resident emergencies; provide first-aid assistance and arrange for appropriate medical attention and follow up.
20. Informs Charge Nurse of changes in the resident’s condition.
21. Attends and participates in in-services and training including 8 hours of Dementia training annually.
22. Other duties and responsibilities related as assigned by Charge Nurse or Assisted Living Director. PHYSICAL DEMANDS OF POSITION The essential functions of this position require the following physical abilities:
1. Standing and/or walking very frequently. Sitting occasionally.
2. Using hands to finger, handle or feel very frequently.
3. Reaching with hands and arms very frequently, pushing/pulling very frequently.
4. Climbing, balancing, stooping, kneeling, crouching or crawling occasionally.
5. Talking and/or hearing very frequently.
6. Lifting up to 50 pounds frequently.
7. Close, distance, color, peripheral, and depth perception in vision; ability to adjust focus.
1. May be necessary to assist in the evacuation of residents during emergency situations.
2. May be subject to frequent interruptions.
3. May be subject to hostile and emotionally upset residents, family members, personnel, and visitors.
4. May be subject to call-back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.)
5. May be subject to injury from falls, burns from equipment, odors, etc., throughout the work day, as well as reactions from dust, disinfectants, tobacco smoke, and other air contaminants.
6. May be subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses.
7. May be subject to the handling of and exposure to hazardous chemicals.
